TeleHealth Appointments Available

There is no doubt that a face-to-face meeting is the best method to complete a functional assessment, but when this is not possible, Telehealth can be used very successfully.  Telehealth uses videoconferencing to deliver health services to those in isolation or those living in regional, rural or remote areas, where travel is not possible.  At Functional Assessments WA, we allow you to choose which method suits you best.  We are happy to complete a face-to-face meeting following the completion of a Covid-19 checklist the day before our meeting.  If we are all well, then we are happy to proceed using social distancing.  If you feel more comfortable with a Telehealth meeting or have a risk factor related to Covid-19, then we can proceed with a Telehealth meeting with ease.  Of course, you can change your mind at any time too.

Functional Assessments
for existing NDIS participants

Whether your current plan has a request for a functional assessment, you are wanting to request an early plan review or you have experienced a significant change in circumstances whereby an early plan review is required, Functional Assessments WA is here to help.
We can assist any NDIS participant over the age of 7 years.

Simply give us a call to talk through your needs or complete our contact form and we will be in touch. Following this initial conversation, should you feel comfortable to proceed, we will arrange an initial planning meeting, ideally in your own home.  At this initial meeting all will be explained, we will answer all your queries and if you wish to proceed, our NDIS documentation can be signed and sorted on the spot to prevent delays.  No charge is made for this initial meeting, so you can make sure you understand the process and feel comfortable with confidence.

As a general rule, we can commence the assessment process immediately following the initial planning meeting and start gathering the evidence required for the report.  At least one face-to-face meeting with the participant is required and all relevant people, you have consented to us liaising with, will be contacted to ensure full understanding of your story or the story of your loved one. The assessment and report generally takes 8 to 12 hours over 1 to 3 weeks to complete, depending on the complexity and availability of those involved.  You will be sent a final draft copy for comment and feedback and when finalised we will send you, the NDIS and your Support Coordinator, if relevant, a final copy of the report.

Many participants already have a therapy provider, say an occupational therapist or speech pathologist, that regularly provides therapy services.  Such providers can certainly provide an assessment report for you, but it is worth considering that sometimes having fresh eyes looking at your whole life story, rather than just focusing on say your communication or social skills, is beneficial to make sure that the right information and evidence is available at your next plan review.  Functional Assessments WA will also provide unbiased recommendations of your needs across the whole spectrum of your life and as we do not provide ongoing therapy, our recommendations are not influenced by future funding possibilities.  This way you can be sure your best interests are being considered.

Functional Assessments
for prospective NDIS participants

The NDIS requires documented evidence from a relevant health care professional to assess your eligibility for NDIS supports.  The best case scenario is that your treating therapist provide such a report, but many people do not receive regular therapy or have a therapist that knows their case or understands the NDIS.  Functional Assessments WA can help you with the evidence you need.
